MESA (Microsurgical Epididymal Sperm Aspiration) is a procedure involving the collection of sperm directly from the epididymis and it is performed in cases of obstructive azoospermia.

The technique involves making a small incision in the scrotum to allow access to the testicle. A very fine needle is then inserted into one of the epididymal tubules that carry the sperm from the testicle to the ejaculatory ducts. Sperm collected in this way can be used for both the assisted fertilisation procedure (ICSI) and cryopreservation.

This technique can be performed under general or local anaesthesia.

It is usually performed in cases of male infertility involving a complete absence of sperm in the seminal fluid (azoospermia).
